Thank you for your interest in the OSB Outreach Department’s services!

In order for us to properly provide services, you must fill out a form based on the services you will need. Form A is for services that are only conducted at Oklahoma School for the Blind. Form B is for services that will be conducted at the student’s home school or at the student’s home. You may only choose one form to fill out.

In addition to filling out either form, please also mail or fax copies of the following information:

Individualized Education Plan (IEP)

Multidisciplinary Education Evaluation & Group Summary
